New Clients
1.Complete Your Headway Registration and Book an Appointment.
We partner with Headway for insurance billing. We must have you complete the steps in Headway to verify your insurance and ensure coverage before your first appointment.
When you click the provider you are seeking care with you will be directed to Headway to book an appointment and verify insurance.
2. Set Up Your Patient Portal Account through Charm
Once you have been verified in Headway you will receive an email from Charm our EHR to create an account for the patient portal. This will allow you to access important documents, communicate with our office, make appointments in the future, access tele-health, and manage your care.
3. Prior To Your First Appointment
All intake forms must be filled out 24 hours before your 1st appointment or the provider may request to reschedule. You can find these forms in the patient portal once your account is set up under the Questionnaires Tab.
Upload any documents or records into the patient portal you would like your provider to have access to prior to or during your appointment.
