Upon scheduling your first appointment you will receive all necessary consent forms through the patient portal (CharmPHR). We require all consents to be completed 24 hours prior to your appointment. Failure to complete may result in rescheduling. 

Upon scheduling your first appointment you will receive via the patient portal (CharmPHR) a copy of AP&W's policies and procedures. Please note that upon signing consents you agree to AP&W's policies and procedures.
